SPED Enrollment Requirements

SPED Pre-Elementary

Report Card or Form 138 (for students who finished Nursery or Kindergarten 1)
Original PSA Birth Certificate / Original Live Birth Certificate
Certification of Good Moral Character (for students who finished Nursery or Kindergarten 1)
(2) pcs recent identical 2×2 ID photo (student, parents and fetcher)
Vaccination Book (photocopy)
Doctor’s Evaluation
(1) pc long white folder
(1) pc fastener

SPED Elementary

ESC Enrollment Requirements

Proof of financial means of both parents or guardian/s, and the other person/s helping send the student to school, if any

Eligibility

• Elementary graduate from a DepEd public school or DepEd-recognized private school

• Incoming Grade 7 student (ALS A&E/PEPT Grade 7 Qualifiers are included)

• Has not been a recipient of the ESC in previous school years

• Filipino citizen (must have a PSA Birth Certificate)

Proof of Financial Means
